This Annex I aircraft form is developed by the Swedish Transport Agency, shall be filled out, and submitted for an initial transfer of a valid class or type from logbook to an Annex I attachment.

This form is also for revalidation, renewal or extension of class or type in the Annex I attachment, and be submitted with the relevant form for the type or class in question.

When the revalidation is based on experience and refresher training in accordance with FCL.740.A (b) (ii) only this form is submitted.
